Join us for our next Sip ’n Support session, focusing on Transitions from Childhood to Adulthood for young people with SEND.

Moving from children’s to adult services can bring lots of questions for families. This informal session will help parents and carers better understand the adult social care system, how the transition process works and what support may be available.

We’ll be joined by Michele Chatburn, AP Social Worker from the Adult Learning Disability Team at Blackburn with Darwen Borough Council, and Khadija Sarwar, Young Adults Worker – Learning Disability & Autism at Lancashire County Council.

What can you expect?

We’ll explore when a young person should be referred to adult social care, who can refer, the referral and assessment process, and eligibility criteria.

Our speakers will also explain the different types of social care support and provision that may be available in adulthood, what families can expect during the transition period, and some of the common challenges they may encounter.

There will be plenty of opportunity to ask questions directly to our guest speakers and find out where to go for further information and support.
