EOL Care Planning training to registered and non-registered Health Care Professionals across the Dartford, Gravesham and Swanley areas.

Training is for ALL Care Providers.

The purpose of End Of Life (EOL) Training is to:

Affirm life and regard dying as a normal process.

Address the whole pathway from recognition that someone may be in their last year of life through deterioration, to the dying phase.

Assess, plan, and provide person-centred end of life care.

Develop communication skills to empower the Health Care Professional (HCP) to initiate and to have conversations with their patients, by involving them and their families in planning for their EOL care.

Enable the dying person to identify their wishes.

Enable HCPs to offer support to patients and their families

To raise awareness of the participants to local services and organisations who may be able to offer support to their patients.

To ensure HCPs are aware of how to refer patients for hospice support.

To ensure and understand medications used to manage symptoms and early planning required to minimize delays in symptoms management.

Provide an understanding of why patients at EOL have swallowing difficulties and how to continue to support their nutritional and hydration needs.

To empower the HCP to be able to support families to understand the signs of deterioration and the importance of comfort and dignity at EOL.

Certificates will be sent within 4 weeks of completing, along with the ellenor End of Life Champion emblem for your strap line etc plus a badge for the learner.

CANCELLATION POLICY. Cancellation/Non-Attendance fee = £100. If you have been granted study leave to attend the training your employer will be charged. Alternatively, if you are attending in your own time, you will be charged. Notification of cancellation must be received 2 weeks before the date of the training.
