From Longevity to Precision Healthy Ageing: A Structured Evidence-Based Approach to Clinical Assessment and Intervention

Longevity science is moving from the search for individual “anti-ageing” interventions towards understanding why people age differently and how this knowledge might extend health span. Genetics, epigenetics, nutrition and lifestyle interact with key ageing pathways involving mTOR, AMPK, sirtuins, autophagy, mitochondrial function and inflammation. Crucially, many of these pathways respond to diet and lifestyle, creating opportunities to personalise healthy-ageing strategies according to individual biology.

This idea underpins precision nutrition for healthy ageing, which aims to combine genomic, metabolomic and microbiome information with targeted dietary interventions. Biological ageing measures, including DNA methylation-based epigenetic clocks, could also help assess responses. However, making a biological-age marker appear “younger” is not necessarily the same as improving healthspan. These tools still need to demonstrate that biomarker changes translate into meaningful outcomes such as reduced disease risk, preserved function and increase health span.

The emerging picture is therefore one of precision longevity built on established prevention. Nutrition, physical activity, early detection and other preventive measures provide the foundation, with genetics, advanced biomarkers, and supplements layered on according to individual need and strength of evidence. Rather than replacing conventional prevention, precision tools may ultimately help make it more targeted
