Presenters: Zoe Harris, Founder and CEO of My Care Matters (which has developed and runs the My Future Care Project), Dani Ayre, Matron and Lead Nurse, Specialist Palliative and End of Life Care Team at Norfolk and Norwich University Hospital, and Clare Fuller, Advance Care Planning Advocate, Power of Attorney Consultant, Educator and Coach
Join Zoe Harris, Dani Ayre and Clare Fuller for an introduction to future planning (incorporating advance care planning). We will explore why making plans matters, how it can help people living with dementia and caregivers - as well as the healthcare professionals supporting them - and the difference it can make during a hospital admission. You will hear about the decisions and choices available, with a particular focus on Lasting Powers of Attorney.
