The online talk takes place on 18 November 2026 at 11:00 and features Hadeel, a senior pharmacist prescriber and clinical academic based in Bradford. She will share findings from her PhD research exploring how older people with polypharmacy self-manage their medicines across the hospital-to-home transition. The session is led by the Research Programme Manager for the SAfe Home Meds Study within the Yorkshire & Humber Patient Safety Research Collaborative.
Short bio : Hadeel is a senior pharmacist prescriber and clinical academic with experience across hospital, general practice and research settings. She is currently the Lead Pharmacist for Education & Training at Bradford Teaching Hospitals NHS Foundation Trust and Research Programme Manager for the SAfe Home Meds Study (SAHMS) within the Yorkshire & Humber Patient Safety Research Collaborative. Hadeel completed her PhD at the University of Bradford, exploring medication self-management among older people during the hospital-to-home transition. Across her clinical, education, leadership and research roles, Hadeel is passionate about bridging research and practice to deliver meaningful improvements in patient care. Short summary: During this webinar Hadeel will share findings from her PhD research exploring how older people with polypharmacy self-manage their medicines across the hospital-to-home transition, with a particular focus on a qualitative study examining patients’ experiences, behaviours and medication-related interactions around discharge. The findings highlight how medication self-management can remain largely invisible within discharge planning and demonstrate how individual, contextual, and system-level factors influence patients’ preparedness to manage their medicines at home. Hadeel will also discuss how these insights informed the development of a person-centred approach to identifying and supporting medication self-management needs during transitions of care.
