Join us for a half-day practical training session focused on effective transitions and reintegration for children and young people accessing Alternative Provision. Transitions can be particularly challenging for young people who have experienced disruption, exclusion, unmet needs or difficulties engaging with education. Effective planning, communication and partnership working are essential to ensure that transitions are positive, carefully managed and centred around the individual needs of the young person. This session will explore how Alternative Provision providers and education professionals can support young people through entry into provision, movement between settings and successful reintegration or progression into their next placement. What will the training cover? Throughout the session, we will explore: Understanding the importance of effective transition planning Supporting a young person’s transition into Alternative Provision Identifying individual needs, strengths and potential barriers Using baseline information and starting points effectively Developing personalised transition plans Building relationships and establishing trust during transition Supporting children and young people who have experienced disrupted education Preparing young people for change and managing uncertainty Understanding when a learner may be ready for reintegration Planning a phased and supported return to mainstream or another education setting Setting clear and realistic reintegration goals Supporting attendance, engagement and emotional readiness Identifying and reducing potential barriers to successful reintegration Sharing information safely and effectively between settings Working collaboratively with schools, Local Authorities, families and other professionals Ensuring the voice of the child or young person is central to planning Supporting parents and carers throughout the transition process Monitoring progress during reintegration Responding when reintegration is not progressing as planned Reviewing outcomes and learning from successful and unsuccessful transitions Evidencing progress, readiness and transition outcomes Practical tools and approaches that can be implemented within your provision Who is this training for?
