British Columbia (BC), Canada declared a provincial state of emergency due to unregulated and toxic drugs in 2016. This declared emergency remains in effect today with toxic drug deaths the leading cause of unnatural death and impacts on life expectancy. Over the past decade, more than 18,000 people have died with unregulated fentanyl becoming the primary driver of toxic drug deaths. During this period, provincial level interventions including expansion of harm reduction (Naloxone, overdose prevention sites, drug checking), prescribed safer supply and decriminalization of currently illegal drugs have been introduced to address the emergency. In this presentation, the focus will be on facilitators and barriers to implementation (including restrictions and retractions over time) of provincial level interventions to highlight key lessons learned in responding to an ongoing public health emergency.
